Total Salpingectomy During Abdominal Hysterectomy Preserves Ovarian blood flow and Function

چکیده
Aim: To evaluate the effect of total salpingectomy on ovarian reserve and function during abdominal hysterectomy. Method: Twenty five patients undergoing total abdominal hysterectomy for dysfunctional uterine bleeding were randomly allocated into two groups; group (I) 13 patients undergoing total abdominal hysterectomy with bilateral excision of both tubes. Group (II) 12 patients undergoing classical method of total abdominal hysterectomy. Antral follicles count, ovarian volume, ovarian stromal blood flow and serum FSH,LH were evaluated preoperatively and 6,12 months postoperatively. Results: A significant increase in ovarian stromal blood flow was oberved 6 months post operatively in both groups with significant decrease in pulsatility and resistance indicies (PI&RI), antral follicle count were significantly higher in group (I) compared to group (II) with no significant changes in FSH, LH levels in both groups. Twelve months postoperatively group I showed significantly higher antral follicles count, larger mean ovarian volume and increased ovarian stromal blood flow than group II. Conclusion: Complete excision of tubes with caution not to damage the ovarian vascular structure during abdominal hysterectomy may preserve ovarian blood flow and function.

Ovarian vein thrombosis after total laparoscopic hysterectomy with unilateral adnexectomy: A case report
INTRODUCTION Ovarian vein thrombosis is a rare but potentially serious complication after surgical and gynecologic procedures such as oophorectomy and hysterectomy. The association of this event with laparoscopic hysterectomy in particular, is very rare. Only two cases have been described so far. PRESENTATION OF CASE We present a case of ovarian vein thrombosis after laparoscopic hysterectomy...
متن کاملEvaluation of ovarian function after hysterectomy with or without salpingectomy: A feasible study
BACKGROUND Prophylactic salpingectomy for the prevention of ovarian cancer has been recommended strongly. The aim of this study was to compare ovarian function in patients who undergo hysterectomy for benign reasons with or without bilateral salpingectomy. MATERIALS AND METHODS This was a clinical-trial study on patients undergone hysterectomy with/without bilateral salpingectomy in Al-Zahra ...
متن کاملOpportunistic salpingectomy for ovarian cancer prevention
Recently accumulated evidence has strongly indicated that the fallopian tube is the site of origin for the majority of high-grade serous ovarian or peritoneal carcinomas. As a result, recommendations have been made to change surgical practice in women at general population risk for ovarian cancer and perform bilateral salpingectomy at the time of hysterectomy without oophorectomy and in lieu of...
متن کاملSalpingectomy as a potential ovarian cancer risk-reducing procedure.
Accumulated evidence over the past 10 years has strongly indicated that the fallopian tube is the site of origin for many high-grade serous carcinomas (1), which represent 50% to 60% of all epithelial ovarian cancers. This multi-origin model of ovarian cancer development has important implications for prevention of ovarian cancer: if some or all high-grade serous tumors develop in the fallopian...

has been increasing evidence over the past decade that the majority of ovarian cancers arise in the fallopian tube and not primarily in the ovary [1-3]. In 2010 the British Columbia Ovarian Cancer Research Group (OVCARE) launched an educational campaign about the potential benefit of " opportunistic salpingectomy " done concurrently with hysterectomy for benign gynecologic conditions, or instea...
